Sildenafil vs Tadalafil: Which Is Right for You?
If you’ve started looking into erectile dysfunction (ED) treatment, two names come up again and again: sildenafil and tadalafil. They’re the two most-prescribed ED tablets in the UK, and they do roughly the same job. The real difference comes down to timing, lifestyle and how your body responds. Here’s the plain-English version, so you can walk into your consultation knowing exactly what to ask.
The 30-second version
- Both are PDE5 inhibitors: they boost blood flow to the penis when you’re aroused. Neither works on its own; you still need to be in the mood.
- Sildenafil (the active ingredient in Viagra) is the on-demand classic. Take it ahead of sex; effects typically last around 4 hours.
- Tadalafil (the active ingredient in Cialis) lasts much longer, up to 36 hours, which is why it’s nicknamed the “weekend pill”. It can be taken before sex or as a regular low dose.
- Neither is automatically “better”. The right pick depends on how spontaneous you want to be, your health and what your clinician recommends.
- Both can interact dangerously with nitrate medicines and poppers. Never mix them.
What they have in common
Sildenafil and tadalafil belong to the same family of medicines: phosphodiesterase type-5 (PDE5) inhibitors. In simple terms, they relax blood vessels so more blood can flow to the penis when you’re sexually stimulated, helping you get and keep an erection.
One thing worth underlining: neither tablet gives you a spontaneous erection. You’ll still need sexual stimulation and arousal for them to work, just as you would without medication. They give your body a helping hand; they don’t take over.
Sildenafil: the on-demand classic
- Also known as: Viagra (and several generic brands)
- Time to work: usually 30 to 60 minutes
- How long it lasts: around 4 hours
- Food: works best on an empty stomach; a heavy, fatty meal can slow it down
- Best for: planned, take-it-when-you-need-it use
Sildenafil has been around the longest and is the one most people picture when they think of “the little blue pill”. It’s well understood, widely used and a sensible starting point for a lot of men.
Tadalafil: the long-lasting one
- Also known as: Cialis (and several generic brands)
- Time to work: from around 30 minutes
- How long it lasts: up to 36 hours
- Food: less affected by what you eat
- Best for: spontaneity, or a longer window without watching the clock
That long window is tadalafil’s headline feature. Because the effect can stretch across more than a day, sex doesn’t have to be scheduled around a tablet. For some men it can also be prescribed as a low daily dose, so it’s simply part of the routine rather than something you take “for” a specific occasion. Your clinician will talk you through whether on-demand or daily makes more sense for you.

| Suits you if… | You like to plan | You want spontaneity |
So which one’s right for you?
There’s no universal winner, and plenty of men try one and then switch to the other. A few questions usually help point the way:
- Do you prefer to plan, or stay spontaneous? Happy to take a tablet ahead of time? Sildenafil is a great fit. Want a longer window so timing isn’t on your mind? Tadalafil’s 36 hours is hard to beat.
- How often are you having sex? If it’s regular, a daily option (only available with tadalafil) can feel more natural than dosing each time.
- How does your body react? Side effects are mostly similar, but they’re individual. Some men simply get on better with one than the other.
- What does your health history say? Other medicines, your heart health and other conditions all matter, which is exactly what the consultation is for.
This is general information, not a recommendation for you specifically. Your Trimu clinician will weigh up your health, any medicines you take and your preferences before deciding what’s safe and suitable. The exact tablet, strength and dose are always agreed during your consultation.
Side effects and safety
Most side effects are mild and pass within a few hours. The common ones for both tablets include headache, facial flushing, indigestion, a blocked or runny nose and dizziness. Tadalafil can also cause back pain or muscle aches, which tend to show up 12–24 hours after taking it and usually settle within a couple of days.
Here’s a quick guide to what’s typical, what’s worth flagging, and what needs help right away.
Usually normal
Mild headache, flushing, a stuffy nose, indigestion or slight dizziness shortly after taking your tablet. These are common, tend to be short-lived and usually pass on their own. Drinking water, resting and avoiding heavy alcohol can help.
Check in with your clinician
Get in touch if side effects keep coming back or don’t settle, a headache lasts more than a few days, the treatment isn’t working for you, or anything just doesn’t feel right. There may be a different dose or a different tablet that suits you better, and that’s an easy conversation to have through your portal.
Urgent: get help straight away
Call 999 or go to A&E if you have an erection lasting more than 4 hours (this is a medical emergency), a sudden loss of vision or hearing, chest pain, or you faint or feel like you might.
Never mix with nitrates or poppers
Both sildenafil and tadalafil must never be taken with nitrate medicines (such as GTN spray or tablets for chest pain/angina) or with recreational “poppers”. Together they can cause a sudden, dangerous drop in blood pressure. Always tell your clinician about every medicine you take, and never accept a nitrate prescription without mentioning your ED treatment first.

Frequently asked questions
Can I switch between sildenafil and tadalafil?
Often, yes. Many men try one, see how they get on, and switch if it’s not quite right. Have that chat with your clinician rather than changing things yourself, so the dose and timing stay safe.
Which one works better?
There’s no single answer. They’re similarly effective for most men; the “better” one is usually whichever fits your lifestyle and agrees with your body. That’s why the choice is worth making with a clinician rather than from a marketing claim.
Can I drink alcohol with them?
A small amount is generally fine, but drinking a lot can make it harder to get an erection in the first place and may add to side effects like dizziness. If you’re going to drink, keep it modest.
Can I take an ED tablet every day?
Tadalafil has a low daily-dose option that some men prefer; sildenafil is taken on demand rather than daily. Whether a daily approach is suitable is a decision for your clinician based on your health and how often you need it.
Do I need a prescription?
These are prescription medicines, so they need a clinical assessment first. (Sildenafil can also be supplied from some pharmacies after a pharmacist check.) At Trimu, a short online consultation lets a UK-registered clinician confirm what’s safe for you before anything is dispensed.
